| Abstract | The medical record is the stage of the process of recording patient data, the data is needed in the flow of health services in hospitals, health centers, and clinics. There are still many health service agencies that still use physical storage media in the medical record process, one of which is the Shipping Science Polytechnic Clinic in Semarang, which results in a lot of data piling up and it's difficult to find the data you are looking for. Thus, the need for recording applications and data retrieval that can help health services. Based on the above problems, the author tries to compile a final project for a web-based medical record application at the Shipping Science Polytechnic Clinic which was built using PHP, MySQL and the Laravel framework. This application was built using the Spiral working method and tested using the black box testing method, white box testing with the results of each function running as needed and a user acceptance test which concluded that users felt 90% of the application helped in the process of recording and searching for the required data. This web-based medical record application is expected to be useful for the Semarang Shipping Science Polytechnic Clinic in assisting the process of health services and assisting health care workers in doing their jobs. |
